To report any adverse effects or concerns about the safety of our medicinal products, please contact us by completing the following form.
How to complete the forms
1º – Click on the corresponding form to open it and then save it (Process only from a PC).
2º – Complete each item of the form in the window that opened.
3º – Perform the process for printing, but instead of using the printer, select the Save as PDF option.
4º – Send the newly created file by e-mail to info@lafedar.com.
